Actually, economics plays a surprisingly small role. Obesity is roughly flat in men across income brackets in the U.S.. And in women it varies more, but less than a factor of two. Japan still wins by a factor of six. (Japan average vs. top bracket U.S..)

Let’s suppose that culture plays a (huge) role — that seems reasonable. If so, what is required for point (1) to remain true?
