I wasn't joking about the cookies thing--I picked a somewhat humorous example because death can be depressing otherwise, but I wasn't mocking the position, but rather illustrating why I don't think it's that much of a problem.
If you stand your ground, from a position of careful consideration, you have a much larger chance of convincing doctors that it's a willing choice rather than a rationalization that you make because you're unwilling to either confront your own mortality or examine your life choices.
