It can be extremely difficult for those raised in American culture to understand the idea that a monarch can honestly and solemnly feel responsibility towards all of their subjects, and a deep sense of duty to advance their interests.
To the American perspective, being a monarch is all about personal privilege--you get the nicest things, people do what you say, you have lots of toys. Me, me, me! And you get all that stuff just because you were born into it!
So it's very difficult for someone with an American perspective to understand a statement like "Queen Elizabeth was a great advocate for Commonwealth countries". When your only frame of reference is personal indulgence, power, and luxury, the statement doesn't even compute.
