Yeah, if a lot of people are lost to follow-up, it would become increasingly implausible that they all detransitioned, making the top end of the range incredibly unlikely. Rather than guessing, though, at some point I think one would just shift the study into "low quality data" w.r.t. that outcome.

A study that cannot control for selection effects just isn't a good one for concluding things about rates.
